Stage 4 Kidney Disease: Understanding eGFR 15–29

Stage 4 kidney disease (CKD stage 4) generally corresponds to an eGFR of 15–29 when the reduction is persistent. This is a severe reduction in kidney function and normally requires close medical monitoring.

Why stage 4 needs careful follow-up

Your kidney team may monitor complications, blood pressure, blood tests, urine findings, nutrition and medicines. Depending on the circumstances, planning for future treatment options may also be discussed.

Symptoms can vary

Stage 4 kidney disease symptoms vary a great deal between individuals — some people have significant symptoms while others have relatively few. Possible symptoms include fatigue, swelling, nausea, appetite changes, itching or breathlessness, but these symptoms can have multiple causes.

Dialysis and transplant discussions

Not everyone with stage 4 CKD needs dialysis immediately. Decisions depend on kidney function, symptoms, complications and specialist assessment. If dialysis or transplant may eventually be required, early discussion can help with planning.
